This page explains how information is handled when you use this website. For questions about patient records or the clinic’s Notice of Privacy Practices, contact your clinic directly.

Appointments and patient information

You can request a visit or ask a general question using our message form. It asks for your name, contact details, clinic and reply preferences, and an optional message (required for general questions). The form runs in our Google Workspace account and sends your submission to info@mtvalleypt.com. Our team uses these details to respond to you; an appointment request does not itself book a visit.

Message contents go directly through the embedded Google form to our clinic’s Workspace email. They are not sent through the website’s application-email service or saved in a website database. The form uses temporary receipt checks and submission limits to prevent duplicate messages and abuse; those checks do not store the text of your message. Emails are retained and handled in our clinic’s Workspace account.

Please leave medical history, symptoms, insurance numbers, and payment details out of website messages. Patient intake and bill payment open MVPT’s existing Prompt pages. For questions about your care, call your clinic. The message inbox is not monitored for emergencies.

Employment applications

Applications submitted here are sent through Resend to careers@mtvalleypt.com, with copies to the clinic’s owners. The email includes the details you submit; the hiring team can reply to the email address you provide. Cloudflare Turnstile helps protect this form against automated submissions.

The website does not save applications in a database. Sent applications are handled by the email service and the recipients’ clinic mailboxes. Unsubmitted entries stay in the open page and clear when you reload or leave.

Keep applications about employment. Please do not include patient information, personal medical information, Social Security numbers, or financial information. This website does not upload résumés or other documents.

External patient services

Review the notices provided by Prompt when using its intake and payment pages. Completing intake does not itself confirm an appointment.

Website operation

Our hosting and security providers may process technical information, such as your IP address and browser information, to deliver and protect the website. External websites linked here have their own privacy notices.
