Patient information
A little preparation.
A clearer first step.
What to bring, what to expect, and the links you need before and after your visit.
Request a visit
Send an appointment request or call either clinic to get started.
Before your visitComplete intake
Complete forms in Prompt when your clinic asks you to. This does not book a visit.
After your visitPay your bill
Pay through Prompt. Call your clinic with questions about a charge.
Before you arrive
Bring the essentials.
We’ll help from there.
- Photo identification and insurance card
- Any referral, surgery instructions, or relevant medical reports
- A current list of your medications
- Comfortable clothing that lets you move
- Your cane or walker, if you use one
Think about the activities you want to make easier and any questions you want to ask. No referral yet? You may be able to begin through direct access. Read the answers below to see how that differs from insurance requirements.
Talk with your clinicThe first appointment
Start with
what matters to you.
Your physical therapist will review your symptoms and health history, then examine the movement, strength, or balance concerns that brought you in. You’ll discuss the findings, your goals, and a recommended plan—including what to work on between visits.
If you need help finding the entrance or have an access or communication need, call your clinic before you come in so the team can discuss arrangements.
